- The patient lives in the United States
- A doctor thinks the patient may have an inherited heart-muscle (cardiomyopathy) or heart-rhythm (arrhythmia) condition — at least one of the following is true:
- The patient is suspected of having, or has been diagnosed with, an inherited heart-muscle or heart-rhythm condition
- A blood relative has been diagnosed with a heart-muscle or heart-rhythm condition
